For this I would need two things: - Some samples of the card entries you want to use to fill the web form. (You can swipe them while on a text editor and send me the files). - The web form itself (so I can ensure that the right fields are filled in from the card). With these I can work on delivering the solution you want on the timeframe proposed (possibly even earlier depending on the complexity of the fields to be filled).
