Bug fix - wordpress user registration - passport upload for verification

I am building a site and the client wants a passport upload option on the user registration page so they can verify the users citizenship. I'm using Sitemile - Auction theme The plugin I have tried is Cimy User Extra Fields. The image upload comes up on the registration page, but the images do not upload. See: [login to view URL] I'm sure this is a quick fix. All mods to be done in the [login to view URL] file so the plugins can be updated. I'd like to also move the [login to view URL] file to the child theme so the theme can also be updated.
