Change the type of file upload box on my site

Hi! I'm having trouble with [login to view URL] I'm trying to convert the books/publications upload box to a series of standard file upload fields. You can see what I mean in the attached images. One is the way things are now- you can see there's just a free text box with some upload options. That's confusing people and not very intuitive. I'd like to change it to something closer to the "DW objective image. Not exactly like it, but one that allows the user to upload certain document types (doc, docx, pdf, odt, things like that) and add a description to them. There needs to be an option to upload multiple files. The files would go where they are now- that is, be visible on the writer's profile page. That's already working right, I just need to change the file upload options that's there now. For example, the writer should be able to upload a document and add a comment. It may be a sample of their horror writing and they can add that comment/note saying "horror writing sample" or something like that. After their account is created, it will be visible on their profile page and potential clients will be able to view the samples. "
