Create a Joomla Page with Form and functionalities (frontend + backend)

Create a framework page based on a ´Joomla Article´ with a FORM, in the which a user can add data in different formats. See the detailed specification in the attachments. Required front-end (design) and back-end programming skills. Important details: 1) All tables in the database have been created already and the specification contains detailed information on which tables and columns to use for each field/function. 2) Page must be responsive and should be based on (and similar to) the existing page: [login to view URL] But with improved layout and actual functions. Use the image in attached func spec as suggestion/guideline onhow improved layout could be. This is the first form/page in a larger project and it is our expectation that we continue to work together on the next tasks if our collaboration and the output is successful.
