PHP Form field capture into database, filtered then posted to remote API server

Hello we have a PHP development project that was started a few months ago then paused cause our parent company reassigned the developer to another project but now need to finish it in order to go live by the end of this week (8/28/2015) The system which was started for us is a PHP lead capture form which captures fields into a database, that data is then "filtered" before being submitted to a third party API portal/gateway. There is also a sort, filter and export to csv php file for the data that was captured and stored in the database for our own internal use. This project is 90% completed and just needs a few things changed, added or tweaked; then we can do final testing and insertion of the form via an iframe into the live html landing page. Please note that if you perform well on this project you will be added to our developer pool of projects which we have a backlog in and given more work based on your skill sets very quickly. Further details and specific questions can be provided/answered via private message etc. A few things we would like to note for this project is: 1) We prefer to work during the hours of 7:00AM to 7:00PM Pacific Standard Time 2) We ask for direct communication with the developer, not "management" or third party "brokers" , "owners" or "bosses" ; working with a middle man causes to many issues, problems and delays. Deception / lying about your skill sets or your role when we communicate with you will result in you being thrown off the project. 3) Transparency, honesty, and integrity; if you are truthful with us and work hard we will always take good care of you! 4) Good backup habits and change control tracking of all files and databases modified in our development environment 5) Detailed documentation of changes you make.
