YouTube Video Intro and Outro - Short

I am needing to have an Intro and an Outro made for my YouTube channel. It needs to be between 4-6 seconds in length for the Intro and about the same for the Outro. Included in the Intro, I need to have a soundtrack that is unique and captivating with a high tempo, very similar tempo as is in this sample provided. I need the work to be able to be easily modified on my end using any of the Adobe products, ie, Premiere Pro, etc. If you are using something other than an Adobe product, please specify beforehand. Things that need to be easily modified on my end is the text, images, and video. You will need to create work with your own sample text, images and video and I will then insert my own information after completion of the project.
