Generate PDF via C# as per the template

I need a function to generate PDF as per the attached image. We need this to be done in either Syncfusion ASP.NET PDF component or open source or RDLC. We have a database and for the records we need to create a PDF as per the attached image. Every PDF page will have header and footer. The record should not cut partially on the page, but move to next page if space is not there, or we can stick to N records per page for ease. Whatever you think is fine. "We want to use drawing tools for lines, text that comes from database, etc." Please note: We do not want Syncfusion's HTML to PDF, nor we want HTML Bitmap to PDF options. I have another alternative if it works for you, then you can drop Synfusion and consider another approach. Another approach is to use Reporting services (I guess it works well with SQL express 2005 with advanced services. Our production server will have Sql server Web edition 2008.). With reporting services, generate report(.rdlc) as desired and then export report as PDF.
