Convert PSD to HTML and CSS

I would like a PSD converted to HTML and CSS. The HTML and CSS has to conform to W3C standards and the website has to work in all major browsers correctly. Where possible all text shall be in text, not pictures. Including navigation. Should be made to the highest quality and in a SEO friendly mark-up. There are 5 pages with similar structure. With a fixed background. Each page is shown as an example here, but I have all layered PSDs and fonts ready to give you. A simple working PHP contact form is needed Please could you also say how much extra you would charge to make the testimonial page editable and what system you would use. Thankyou for your time.
