The purpose of this website is to get the visitors to fill out a form that will be stored in a MySQL database and a message notification with all the information sent to various email recipients. You will receive the content for 3 pages HOME - ABOUT US - HOW IT WORKS from me. It is a professional marketing site. All graphics must be original and/or not copyrighted to someone else. Website must have a graphics header, a footer for copyright information and other visual aids to help the visitor navigate to, and fill out the form. After submission, the visitors will be allowed to view a report on .pdf format for download or webview. ## Deliverables 1) Complete and fully-functional working website with all source code. 2) Deliverables must be in ready-to-run condition, as follows: Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
