Contact US page on Volusion - CRM

We currently have an osCommerce website and we're switching over to Volusion. We'd like someone to set up our Volusion CRM so that when someone goes to the "Contact Us" page, they can fill out a ticket that goes into our Volusion CRM. We'd like the page to look similar to our current contact us page, which has our address, phone number, hours of operation, etc. Work must be well documented, meaning you must submit a list of all pages that you change, what changes were made, and where the pages are located in the admin or on the FTP. NOTE: If you're a Volusion expert, we'd definitely like to hear from you! There are quite a few things we'd like done on our new Volusion site before we can turn it on, and we'd love to find someone with the expertise to handle it all.
