Setting up a database

I am in a tight spot. I need a database created to collect data generated by this page: <[login to view URL]~yourtota/[login to view URL]> There is no database right now. Everything will be done from scratch. I would like to have this job done by 3:00 PM Sun Dec 26 (Eastern Time) if possible.
