Set up data base and CRM for new website

I want to set up a database/CRM for my new web business, and also need some PHP programming. My website is www.hdhomeview.com. I am using GoDaddy with MySQL Database on a Linux hosting account. I have several requirements. Redirecting on April 3, so it may be down for a few hours. I have an Excel spreadsheet that includes the basic data I want to manage. This does not include the shopping cart and billing functions which I also want to add. I need to initiate some kind of CRM system. Looking for advice on this. I have a PHP form at that gathers information from customers. I want to integrate this into a CRM program, which I need to select and set up. I am also going to be adding a Viewing Feedback form that I want to create (in PHP?) that has a few special requirements.
