Brochure request system - email delivery + labels > pdf

The project is to create a PHP form for users to use to request a brochure, for a salon. There will be two options - delivery by post, and delivery by email. If the user selects delivery by email, our PDF brochure will be sent to them via email, and their details recorded. If the user selects delivery by post, their name & address will be stored in the database, ready to be printed by the administrator. The admin panel will contain a screen that shows pending brochures. Clicking "print" will export the addresses to a PDF (we already have a PDF class - ezpdf - and even a label class, "clabel.ezpdf") - so basically, it's just passing the relevant values to the PDF creating class we already have. You will be expected to thoroughly annotate and document your code, and are asked to keep it as simple as possible. Please see the attached document for an exact specification.
