i want to build a contact/order form which will allow my visitor to specify what they need i have examples i can show you i offer iphone screen repair service and this is what i want the customer to be able to do 1. select make of device 2. select model of device 3. select color of device 4. select what they need repaired 5. select if they want to "Drop-off device", "have device picked up and dropped back off", "On-Site repair" 6. input contact & payment information 7. place order and get order number i want the UI to be very easy.. i can show you example
