Survey digitizing application

An application on Windows to use a wintab standard digitizer to build a profile of a survey score sheet or questionaire (1 or more pages), and then extract the survey data. The profile must be stored and retrieveable later. Editing of the profile is not a requirement. Profile data to include: field location, field data type (alphanumeric, numeric, check boxes (single or multiple selection) and line scale. Some fields may be entered once and then repeated as part of the surevey sheet. Using the profile and the digitizer extract survey data from marked score sheets. Input of numeric values via a numeric pad on the digitizer (the digitizer or a virtual one). Once a survey sheet is completed, present a summary of the data and confirm data save or redo of the current sheet via the digitizer or keyboard. The initial data output format will be an ascii flat file of 80 columns with fixed field widths. This is expected to change and the application must be designed to allow for additional output formats to be added. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. ## Platform Windows 2000 and XP
